Creating a form
Firstly, login and access the manager dashboard. Scroll down to organisation settings on the left and select Forms.
Click on the New Form button.
You can choose to start from a template form or to start from a blank form.
Enter the name of the form. Note: This name will be visible to the end-user of the form.
Enter an email address that will be notified when forms are submitted. If you do not want to be emailed, please leave this blank.
Select if you want the form to have a single page, or multiple pages.
The design section is where you insert the questions and information you want on your form.
It works in a drag and drop style.

Depending on the types of forms you need to create, you may want to add columns in, to better format your forms.
Add in the questions you need.
We recommend scrolling down to update form often so that your work saves in case your internet drops out.
