Do I need a Lifecard account to do an online form?

Yes, you need to register for a Lifecard account to complete an online form.

If you do not wish to do so, you can request a paper version of the form. 

You can also delete your Lifecard account after your admission - noting that you will lose the form submission and will have to re-enter all your information next time you visit.
